Support for Primary Health Care (POZ)

Primary health care is part of the health care system, which provides all persons entitled to benefits residing/staying on the territory of Poland with comprehensive and coordinated health care services at the place of residence. Benefits are provided in outpatient conditions (in the office, clinic or clinic), and in medically justified cases, also at the patient's home (also in the social care home). Kohesio is the European Commission's open-data platform for projects co-financed by the European Regional Development Fund, the European Social Fund Plus, the Cohesion Fund and the Just Transition Fund.
